Tooth caps on teeth with fillings?

Does it hurt when they file down your teeth to put a cap on it? Especially if it was a tooth that has a filling. The idea of filing a tooth away is painful enough but if it is sensitive due to a cavity that makes it worse right? I know you get Novocaine but still I'm sure you can feel it? Thank you!
