Water boiling out of reservoir on my 1995 Honda Accord?

within 10 mins. of driving my car I can hear sizzling sounds coming from under my hood. When I opened it I saw water sizzling out of my reservoir cap, no where else. Also my gauges started going up showing my car was over heating. Sometimes it will show it over heating, however, there are times the water will be coming out with lots of pressure out of the reservoir but my gauges don't show it over heating. Please any suggestions will help..thank you...
